Description:
The format tool when Modifying the Bibliography format is broken. If you add an item it will not insert it, then displays the incorrect value.

Steps to Reproduce:
1. Insert->Table of Contents and Index->Table of Contents, Index, or Bibliography
2. Change Type: to Bibliography
3. Select Entries Tab
4. Change Structure and Formatting

Add and delete items.

Actual Results:
The fields get smashed up, then don't show in the Bibliography.

Expected Results:
It will be obvious how it should work.
