Description:
It is possible to add toolbars to documents (rather than Writer), and have those toolbars remembered with the document. It is also possibile to associate an icon with a toolbar entry (it works fine while the document is open).

When the document is closed an re-opened, the toolbar is remembered, but all the items have lost their icons and reverted to text.

Steps to Reproduce:
1.Create a document in writer.
2.Add new toolbar to the document.
3.Add item to toolbar
4.Customize the item to use a standard icon
5.Check that it is displaying correctly.
6.Save and Close the document
7.Re-open the document
8.Note the missing icon

Actual Results:
Icons lost

Expected Results:
Icons should still be present

I don't know how to do this. Could you please give me an explanation?

(i) Right-click on your new toolbar, select "Customize Toolbar"
(ii) Pop-down 'Scope' (top -right of dialog) and select the document
(iii) Choose the toolbar you created in the 'Target' drop-down
(iv) Select anything in the 'Assigned Commands' list
(v) Pop-down "Modify" (below the list of assigned commands)
(vi) Select "Change Icon"
(vii) Pick any icon
(viii) You should already see the icon has changed
(ix) Click "OK"

All works fine. Now save the document and reopen; icon will be lost.
