(i) Right-click on your new toolbar, select "Customize Toolbar" (ii) Pop-down 'Scope' (top -right of dialog) and select the document (iii) Choose the toolbar you created in the 'Target' drop-down (iv) Select anything in the 'Assigned Commands' list (v) Pop-down "Modify" (below the list of assigned commands) (vi) Select "Change Icon" (vii) Pick any icon (viii) You should already see the icon has changed (ix) Click "OK" All works fine. Now save the document and reopen; icon will be lost.
