Description:
Graphic in the base form document window is not readable

Steps to Reproduce:
1.call forms window in BASE module
2.click document (right of the indicated forms)
3.--> a destroyed (iconed) graphic of the selected form will be shown right of the selected form

Actual Results:
destroyed graphic

Expected Results:
small version of the affected form

Could you pleas add a screenshot which shows the destroyed graphic? I don't know which graphic you see there.

Have got a screenshot per private mail. This bug is a duplicate of https://bugs.documentfoundation.org/show_bug.cgi?id=121995
