Description:
ALT+ENTER before Table of content does expand the TOX with additional field 

Steps to Reproduce:
1. Open the attached file
2. Press ALT+Enter

Actual Results:
Paragraph before, but in field formatting (and badly drawn)

Expected Results:
Previously ALT+ENTER didn't work. It wasn't possible to create a paragraph before Table of content if that was on the start of the page at all

Tested with attachment 177056 [details] from bug 146356

Additional informations
- "update index" deletes additional field
- Customise dialog shows no command assigned to ALT+Enter
- If you assign a command to ALT+Enter command does't work at beginning of TOC (but outside TOC)

This is fixed with the patch of bug 146356
