Form with allocated fields to fill in data

With just 1 field entered in the attached .docx form, Writer gives a 
"LibreOffice 3.4 has stopped working" message - A problem caused the program to stop working correctly.
Happens every time.
The form is attached.

NOT Reproducible with reporter's sample document and "LibreOffice 3.4.5 RC1  - WIN7 Home Premium (64bit) German UI [Build ID: OOO340m1 (Build:501)]"

Confirming LibreOffice 3.4.3 (OOO340m1 Build 302) running under Windows 7 Home Premium. English UI
After opening the document, I typed "Lyme Bay" (without quotes) into the first shaded box and selected Save As. I changed the name by adding 1 at the end of the name and clicked Save. Got the Stopped Working message as described in initial post.
Reloaded LibreOffice and repeated by typing 54 into second shaded box and clicked Save. Same result. Reloaded LibreOffice and repeated by typing 2011 into third shaded box and clicked Save. Same result.

[Reproducible] with "LibreOffice Daly based on 3.4.2 RC - WIN7  Home Premium (64bit) German UI [OOO340m1 (Build:201) from libreoffice-3-4~2011-07-22_15.35.00_LibO_3.4.2rc1_Win_x86_install_multi.exe]"  from 2011-07-23

No problem with "LibreOffice 3.4.5 German UI [Build ID: OOO340m1 (Build:502)]" parallel Server installation on German WIN7 Home Premium (64bit) 
No problem with "LibreOffice 3.5.0 RC2 German UI/Locale [Build-ID: e371a95-bf68a13-5a1aa2b-d3c1ae9-b938258] on German WIN7 Home Premium (64bit)

Please feel free to reopen this bug if you find out that the problem still exists with the current stable LibreOffice version (3.4.5)!
