I thought of yet another improvement: 3. Provide serial numbers to all changes in the "Accept or Reject changes" window. Now when the user hovers his mouse on a change, display this serial number as a tooltip. This will instantly establish a correlation between the change in the document and its entry in the "Accept or Reject changes" window. This would be so great to have, I agree. Right now, finding where one is when you move around in a revised doc in a non-linear fashion is terribly unintuitive! Enhancement request.
