I've found out that, after closing a document I'm writing in Writer application, when I open up again the same document, it's showing the two last notes I have created as blank notes, without date nor author, and if I click on the notes and then out, is automatically excluded. It happens only in one of my documents, and it very odd. 

Please let me know if I can be of any further help. Unfortunately, I can't see any odd thing in my system to report. I'm using LibreOffice like any other day.

Hi, could you provide the document with which it happens? If you add notes again to the document, are they also altered? Thanks - Sophie

Hi Sophie, I can't send you the document, since is produced as part of my job for a government company and is for internal use only. It would be useful if I send you the document without it's content?

I've edited the document during this morning and haven't noticed any problem. Others notes were created, and the bug isn't occurring again.

In this case I would propose to close this issue.  In case it occurs again and is reproducible then it could be reopened.

by the way you should consider upgrading to a more stable 4.2.x release like 4.2.6.2 which will be released tomorrow.

the 4.2.0.4 version you reported the bug with is a very immature early 4.2.x version
