The image sx03242.png is not delivered in any of the image themes. It is used in the help in file /text/shared/01/01010000.xhp, which has the title "New" and belongs to the drop-down-list File > New.
The image is referenced as "galaxy/res/sx03242.png".

The image exists in /core/icon-themes/galaxy/res, but it is not packed into the delivered images_galaxy.zip or any of the other delivered themes.

To see, that it is missing in the help, search for "New" with "Find in headings only" and "Complete words only". Use the search result "New".

I see missing file for Templates icon.
