The likely place for a chapter index is at the beginning of that chapter. The Heading of the chapter then is repeated in the first line of the index. I would like to avoid that repetition and suggest not to include the chapter heading in a chapter index or to make inclusion optional. If excluded the user still has the option to write an appropriate text in the index title.

Disagree with the need to fiddle around with outlines. You better create another style that looks alike your headings but uses text body as outline level. Just create a new style based on the heading, eg. below H1..9 to inherit all attributes, and double-check the Outline & Numbering tab.
