Description: in a writer document, open the document converter function, choose something to go to the next step. Now in the select path you can see that the "create new folder" icon is missing.

Step:
-Create a new writer document.
-Go to file > Wizard > Document converter.
-In the document converter choose something and click next.
-Open the select path (the button with three dots).
-Check that the icon is missing.

Can not confirm on Windows 8 Ent 64-bit en-US with
Version: 5.1.4.2 (x64)

With Tools -> Options: LibreOffice General dialogs "Use LibreOffice dialogs" checked active. The icon for the "Create new Folder" button is visible. And the 

Is the missing icon a Linux distro/DE issue? Even a GTK+2 vs. GTK+3 theme issue?

On Windows builds--using Windows dialogs rather than LibreOffice's--the dialog to show the "Select path" "Please select a folder" chosen seems a poor GUI layout.

Repro only with KDE backend. Gtk3 shows it.

I can partly confirm it with

CPU Threads: 4; OS Version: Linux 4.5; UI Render: default; VCL: kde4; Layout Engine: new; 
Locale: de-DE (de_DE.UTF-8); Calc: group

If I start LO "SAL_USE_VCLPLUGIN=kde4 /path/to/soffice", I do not see the icon but get a quick help there with "Create New Folder". Clicking on that empty space opens a new dialog to enter a name for your folder.

Starting without this "SAL_USE..." (or changing to "...=gen" or gtk2|3) however does show this icon.

Could you give a try with a recent LO version?
BTW, kde5 is gonna replace kde4 rendering in LO.
