The index nesting is messed up in Chapter 1 and Chapter 2, as follows:

* Chapter 2 is nested inside Chapter 1, rather than a sibling.

* Chapter 3 is nested inside Chapter 2, rather than a sibling.

The result is that folding up Chapter 1 (clicking on the index down arrow) folds all other chapters, but only chapter 1 should get folded.  Likewise with chapter 2.
