Slides that are created through the outline mode don't have footer.
More precisely, the View -> Header and footer popup show that their "date and time" and "footer" fields are empty, and "slide number" field is not activated.

Tested configurations:
LibreOffice 4.2.8.2 under Linux Ubuntu 14.04
LibreOffice 5.1.5.2 under Windows 8
LibreOffice 5.1.4.2 under Linux Ubuntu 16.04

How to reproduce this bug :
- Create a new Impress document (no template needed)
- Add to the empty slide a footer : View -> Header&footer... enter a date, a footer text and tick the slide number option. Apply to all
Now the (only) slide has a footer with these 3 pieces of data.
- Enter the outline mode (View->Outline)
- Give the slide any title, and press Enter to creat a new slide
- Go back to normal mode (View->Normal) : the new slide hasn't any footer
