Can't add a form control to a spreadsheet.

No crash for me. Have you tried it in Safe Mode?

No repro here either with 6.0.0.2 / Windows 7.

I see "(Web)" in the title, how do you get in that state?

Don't know how I got there. But it crashs anyway.

No crash in Safe Mode.

After reseting the user profile everything is fine.
