Form that will not render correctly.

This problem relates to LibreOffice when importing a PDF document into LibreOffice Draw.
It pertains to:
LibreOffice Version: 5.1.6.2
Build ID: 1:5.1.6~rc2-0ubuntu1~xenial3 (Ubuntu 16.04LTS)
CPU Threads: 4; OS Version: Linux 4.4; UI Render: default; 
Locale: en-AU (en_AU.UTF-8); Calc: group

Having experienced this problem, I then loaded Windows 10 into Oracle Virtual Box and installed the latest LibreOffice Suite.  
Details of this LibreOffice are:
LibreOffice Version: 6.0.2.1 (x64)
Build ID: f7f06a8f319e4b62f9bc5095aa112a65d2f3ac89
CPU threads: 1; OS: Windows 10.0; UI render: default; 
Locale: en-AU (en_AU); Calc: group

Both operating systems have been updated to the latest buildstate.

Both versions in both operating systems experience the same problem with rendition of the PDF document.  Specifically the green rectangular background becomes skewed at an angle of approximately 30 degrees.  The second page has a similar rectangular green background but it renders properly in both operating systems and both versions of LibreOffice.

This problem is repeatable.

Of interest this document renders properly in the Document Viewer 3.18.2 which comes with Ubuntu 16.04LTS

I would have liked to have provided you with a snapshot of the document as it renders in LibreOffice but this bug reporting system appears to only allow for one attachment.  With that in mind, I feel that providing you with the actual PDF document is likely to be more helpful in enabling you to investigate this bug.

The original PDF form is attached as "Conference_Registration_Form_2018.pdf"

1. Download and install oldest version of LibreOffice (usually 3.3 unless your bug pertains to a feature added after 3.3) from https://downloadarchive.documentfoundation.org/libreoffice/old/

2. Test your bug
3. Leave a comment with your results.
4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
4b. If the bug was not present in 3.3 - add 'regression' to keyword
