Human icon theme was removed a long time ago: https://wiki.documentfoundation.org/ReleaseNotes/5.3#Feature_removal_.2F_deprecation Please use a maintained icon theme. Closing.
if it was removed, why is it the default on a new system using the most recent ppa version of the latest libreoffice release? there seems to be another issue here then.
