To reproduce:

Click on Insert->Envelope->Format
The format generator requires the right edge of the Sender area to be to the left of the left edge of the Addressee area.

Since I have an envelope with two windows, one directly above the other, I can't generate an appropriate window format.

A screen shot of a typical (unusable) format is attached.

The values that I need (but which can't be entered) are:
Addressee
   Position 
      from left  0.5 "
      from top   2.6 "
Sender
   Position 
      from left  0.5 "
      from top   0.7 "
which Libreoffice won't let me enter.

I suggest a format generator which allows full specification of the two windows, say the upper left corners (as now) but also the lower right corners.

Confirmed on 7.1.7 for Mac OSX 11.6

Also it is not possible to format a DL Envelope to meet the requirements for the Royal Mail in the UK.

In my opinion I see little value in the envelope wizard et al. Format a page to the correct dimensions and insert frames where you want the text to be. I say frames rather than text boxes as frames can have mail merge and data base fields where as text boxes cannot. Frames can then be placed wherever desired on the envelope. Then do two merges. On for the envelope and one for the document to enclosed in the envelope.  It has to be 2 merges I believe as LO Writer cannot handle the printing of more than the first defined page size.

4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
