Radio button issue

1. All radio buttons are checked but only one should.
2. The lengthy label breaks the UI; we could make it shorter or put the radio buttons below (but indented as the other input controls)

Besides, does the "QR Code" title make sense? How about Parameters or Options...

Options sounds better and less technical. 
Where is the QR option in order to generate one?

Shubham Goyal committed a patch related to this issue.
It has been pushed to "master":


tdf#126720 Fixing label length fix UI break

It will be available in 6.4.0.
