Description:
it has been forgotten to call QR code application

Steps to Reproduce:
1.run openoffice-writer
2.insert
3.objekt
4: ..... Missing QR-code function

Actual Results:
no

Expected Results:
please try it out briefly, then it is immediately visible

Can not confirm on Windows release build of 6.4.1
Version: 6.4.1.2 (x64)
Build ID: 4d224e95b98b138af42a64d84056446d09082932
CPU threads: 8; OS: Windows 10.0 Build 18362; UI render: GL; VCL: win; 
Locale: en-US (en_US); UI-Language: en-US
Calc: CL

On this platform/build, the QR code dialog behaves as documented in Help [1].

Please provide details from the Help -> About dialog. And also your os and Desktop in use.

OK, thank you for the Team Viewer screen capture, took a moment to figure out how to open ;-)

Anyhow, I assume you did an upgrade installation and not a new installation of LibreOffice 6.4

Could you please relaunch:

   Help -> "Restart in Safe Mode..."  and Restart

   on relaunch, the "Safe Mode" dialog will show. Click  "Continue in Safe Mode"

That will bypass settings in your user profile.  

If the Insert -> Object -> QR Code is restored, then issue is in your old user profile.  The old profile can be simply deleted, or you can export and copy out any customizations you need to keep (should not be many).

That will show the LibreOffice build details for your Windows install.
