Please provide details from the Help -> About dialog. (In reply to päde from comment #2) > Created attachment 158469 [details] > missing QR application OK, thank you for the Team Viewer screen capture, took a moment to figure out how to open ;-) Anyhow, I assume you did an upgrade installation and not a new installation of LibreOffice 6.4 Could you please relaunch: Help -> "Restart in Safe Mode..." and Restart on relaunch, the "Safe Mode" dialog will show. Click "Continue in Safe Mode" That will bypass settings in your user profile. If the Insert -> Object -> QR Code is restored, then issue is in your old user profile. The old profile can be simply deleted, or you can export and copy out any customizations you need to keep (should not be many). Let us know.
Also, please post the text from the Help -> About dialog, as I had done in comment 1 That will show the LibreOffice build details for your Windows install.
