Steps to Reproduce:
Steps how to reproduce with Version: 6.4.2.2 (x64)
Build-ID: 4e471d8c02c9c90f512f7f9ead8875b57fcb1ec3
CPU threads: 4; OS: Windows 6.1 Service Pack 1 Build 7601; UI render: default; VCL: win; 
Locale: de-DE (de_DE); UI-Language: en-US,  Tango theme

1) Launch LibO
2) Menu ˋFile → Openˊ  
   » LibO File dialog with lots or prepared locations opens 
3) Click one of the several ftp locations (some of them do exist, some are
   obsolete)
   » Authentication dialog appears
4) Click Close-X every 5 seconds
   » Authentication dialog reappears on and on 
     Finally LibO crashes 😥

So let's set status to NEEDINFO meanwhile. I could see that there are only 19 crash reports with that signature [1] and all with LO 6.4.
