Description:
I add a picture to a file, 2-10 in document; the menus get greyed out, I can not click, I save and close - all pictures have been replaced by the same picture!! Very annoying, 5th time today this happens in odt file! (new file!)

Steps to Reproduce:
1.Open file
2.Add pictures (edit - picture - jpeg)
3. Write some description, add more pic, wait ...

Actual Results:
most or all pictures have been replaced by the same picture

Expected Results:
The pictures I placed in the document should be in the spot I put them

Reproducible: Always

1) Does resetting the user profile make a difference:
https://wiki.documentfoundation.org/UserProfile/de

2) The second step would be updating to 6.4.5.2 (or is the version field incorrectly showing 6.3.6.2)

3) The third step would be sharing the bug doc and some images to attempt to reproduce the issue
