Open the attached writer file, right-click the TOC and chose "Edit Index". 2. Chose the Entries Tab and select Level 4. Make some changes if desired. Close the dialogue. 3. Reopen the "Edit Index" dialogue. EXPECTED RESULT: The dialogue should open on the Entries tab at level 4. ACTUAL RESULT: The dialogue opens correctly on the Entries tab but always defaults to Level 1.

Yes, to remember the last user choice is desirable. But a minor issue in this special case. And rather an enhancement IMHO.
Andreas Heinisch committed a patch related to this issue. The patch should be included in the daily builds available at https://dev-builds.libreoffice.org/daily/ in the next 24-48 hours.
