Don't thinks this needs UX advise, but always nice to have Heiko in the loop

What do you expect? Stretched (onto the page background) means the image to be distorted and the preview shows this.

Use Custom position/size and large values for width/height to proportionally scale. Or tile to fill the entire background.

Nevertheless, Area Fill > Bitmap has not been finished, see bug 113224. The original idea is described here https://design.blog.documentfoundation.org/2015/12/22/area-fill-options-made-consistent/ (pretty sure there is a duplicate to this issue).

If you check stretched the preview shouldn't stretch?

Hmm, the preview should match the actually stretchiness on the document itself. Wanted to check that, but ran into: bug 141059

Let's close this ticket. Some possible configurations haven't been implemented, see bug 113224, but the status of stretched images is correctly shown in the preview (all options are disabled but per custom position/size and scale you get the same result).
