1. Mail Merge Wizard
2. Select Starting Document: Use the current document
3. Select Document Type: Letter
4. Insert Address Block: 
   1. Address list: In my case a .xlsx-File consisting of several tables
                    (the converted .ods-File gives the same result)
   2. Address Block (New...):
                     -Company Name -> insert
                     -Address Line 2 -> insert, move to next line
                     -Title -> insert, move to next line
                     -mouse click in the upper window ("1. Drag address elements
                       here"), to the right of <Title>; <Title> is no longer
                       marked and you can see the cursor
                     First Name -> insert

Result:
In my case the third line looks like this:
<T<First Name>itle>
The preview looks similar whether the fields are already matched or not.

I tried it on:
Linux Mint 20.1 Cinnamon; LibreOffice Version: 6.4.6.2
Xubuntu 20.04.2; LibreOffice Version: 6.4.6.2 and 7.0.4


You can avoid this bug making sure one of the already included elements is marked. And for some reason it doesn't occur in the first line (second line gives similar result).

I reproduced in 7.2+ so I'll set New. 
No special source needed,in Insert Address Block just use More - New. 

But it needs more tries in adding elements, not as simple as written. 
Reporter, please try to find exact steps and trick that produces bug reliably.

1. Download and install oldest version of LibreOffice (usually 3.3 unless your bug pertains to a feature added after 3.3) from https://downloadarchive.documentfoundation.org/libreoffice/old/

2. Test your bug

4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
4b. If the bug was not present in 3.3 - add 'regression' to keyword
