Description:
I was sent the attached request form to complete. In LO writer the boxes are in the wrong place. They open correctly in Word 365 - I exported as a pdf from Word to show how the documents should look.

Steps to Reproduce:
1.Open the attached .rtf

Actual Results:
In LO writer the boxes are in the wrong place. They open correctly in Word 365

Expected Results:
Boxes in correct place


Reproducible: Always

Looks good in LO 24.8.0.0.alpha0+ (ef6083200a4f28e43198c7a0878da6f4b880725f) / Ubuntu.

This got fixed in multiple steps, with the final one being the following in 24.2, also backported to 7.6:
