Description: I was sent the attached request form to complete. In LO writer the boxes are in the wrong place. They open correctly in Word 365 - I exported as a pdf from Word to show how the documents should look. Steps to Reproduce: 1.Open the attached .rtf 2. 3. Actual Results: In LO writer the boxes are in the wrong place.
