Could you please paste the info from Help - about LibreOffice ?

I have set the bug's status to 'NEEDINFO'. Please change it back to

I have provided the version and also file are given can check and open the same in libreoffice .

I tried to open this in MS Word 2003, but it looked awful. Can we have a screenshot or PDF of what it should look like?

Looks correct. please test with newer version of LibreOffice.
