On which LO version are you? Could you try last LO version 7.4.5 + apply https://wiki.documentfoundation.org/QA/FirstSteps ?
Also, do you use ARM or x86-64?
@nik : does it do this for all ODS files, or just particular ones called "Music...something" ? Does it make any difference depending on where you store the ODS files. I can not reproduce this with Version: 7.4.4.2 / LibreOffice Community (aarch64 Arm M1) Build ID: 85569322deea74ec9134968a29af2df5663baa21 CPU threads: 8; OS: Mac OS X 13.0.1; UI render: Skia/Raster; VCL: osx Locale: fr-FR (fr_FR.UTF-8); UI: fr-FR Calc: threaded 1) Created a test Calc file called "Music Listing.ods". Filled cell A1 with random character string. 2) Saved to Downloads folder. 3) Icon displayed is green rectangle with white grid and pale green chart overlay. 4) Moved the ODS file to different locations on the hard disk, no difference or change in icon representation.
@nik : you appear to have an app installed in your system that has replaced the default icon for Calc spreadsheets. You can find out which app this might be, by selecting an ODS file, pressing the Cmd-I keys, and checking which application is used by default to open the ODS file ("Open with" section of the file properties dialog). That should at least give you a hint as to where things have gone wrong. You can reassign this globally for all ODS files by choosing the appropriate LibreOffice application in the list of registered applications, and clicking on the "Change for all" button, but you need to have admin rights to do so, and generally you will have to unlock the properties dialog via the admin password in order to do so.

You can also reassign the icon of your choice to the unwanted icon via the same file properties dialog, copying the icon representation that you want, and pasting it over the topmost icon in the file properties dialog.
Further information : The icon @nik posted doesn't appear in the default systems icons of macOS, which are to be found here : /System/Library/CoreServices/CoreTypes.bundle/Contents/Resources/ This would indicate that the icon in question comes from a third party application that has assigned its own icon to LibreOffice ODS files. If this is verified by Nik, then it would not be a LibreOffice bug. I also can't see any such icon in the LibreOffice resources folder: /Applications/LibreOffice.app/Contents/resources/ It is possible that an extension has added this icon, but in that case, Nik would need to check whether any LibreOffice extensions have been installed, and whether any of these might have that icon. Either way, we need more information.
Nik has found a way to correct the problem by activating native LO dialogs, so we can mark this as WFM.
