1. View - Toolbars - Tools, click on dropdown arrow for first icon.
Hover mouse over last two icons in last line of dropdown menu.
Notice that the icons are identical, but refer to different dialogs
"Insert Table of Contents, Index, or Bibliography" (.uno:InsertMultiIndex) and "Insert Index Entry" (.uno:InsertIndexesEntry)

2. Can also be seen from menu with:

Insert - Table of Contents and Index - (submenu)

3. Finally, use Tools - Customize - Menus tab (search on: Index)

Notice that "Update All" (.uno:UpdateAllIndexes) also has the same icon.
(looks like .uno:InsertMultiIndex icon is linked with .uno:UpdateAllIndexes
Perhps .uno:UpdateAllIndexes should have a "reload" symbol on it, to differentiate it from the "Insert ToC, etc." icon.

ToC (left), Index (right) in Word 365

In Word 365, both function (Insert ToC and Insert Index) has similar icons. Currently I don't have any idea how to differentiate between two.

rough idea for index entry icon

In Tools - Customize  - Menus tab, search on: index
(this should show all the relevant .unos/icons)

My proposal:

.uno:InsertIndexesEntry   (see attachment)
.uno:InsertMultiIndex     [no change]
.uno:UpdateAllIndexes     add a circular (update) arrow in bottom right corner


Comments:
1. Probably the ToC icon could be improved -- but for now "no change".
2. The attached proposal for Insert Index Entry:
   a. should not have a bent corner (like ToC icon), 
   b. should only have text lines, not a heading-like top (like the ToC icon).
   c. the "blue" box in the middle, among three lines of text, gives the same visual impression as when this dialog is used (and "Show Field Shadings" is enabled, Ctrl+F8).
  To see the actual behavior of Index Entry:  select some text, Insert - Table of Contents and Index - Index Entry, Insert, Close.   (if you do not see the shading, then press Ctrl+F8)
   d. to be clear, the attachment is only to communicate the "concept" or "idea" of the icon. I am sure you will adapt it to fit with the themes.

The icons in Karasa Jaga

I can't see clearly your rough design, but would the design concept of the Karasa Jaga icon be successfully reused by Colibre?

Another thing that needs to be checked is how it looks in all icon themes. Also it's better to mention which icon you are having trouble with. I always assumed your reports were about the Colibre icon theme.

Colibre, Karasa Jaga and Sukapura index icons

You are right Rizal, I had only looked at the Colibre icons when I opened this ticket, but now see attachment with Colibre, Karasa Jaga, and Sukapura.

1. The Karasa Jaga "index entry" icon is more or less the same as my "rough idea", with one difference.  The KJ icon looks like bulleted lists.

I would make the lines be complete (like a document page), and then it looks like the KJ icon has a blue box in the middle -- which is also what I was trying to suggest. 


2. The KJ ToC icon is different from the other KJ index icons, but the blue boxes on the right side do not make much sense.  I would consider something like:

▪-----▪

Where the idea is to be closer to a Table of Contents, like:

[Heading Number] text entry [page #]

3.  Maybe you can see now in the attachment that Update All should also be differentiated.
