A sample document is not required, just create a new page style.
He will disappear on his own
I will also attach a simple picture.
Образец документа не требуется, достаточно создать новый стиль страницы.
Он пропадёт сам
Также приложу простую картинку

Open attached document. Save it as docx. User defined page styles have been gone.

I could confirm the buggy behavior. Have created a little example file with 2 user created page styles.
Saved in *.odt, closed and reopened and styles have been saved too.
Saved as *.docx, closed and reopened and styles are gone.
Note: The "default" page will now look like the user defined style I had set for this page.

Steps:
- Open the document form comment 6. Go to Sidebar - Styles - Page Styles - apply "Used Page style" OR "New Page style". Everything OK.
- Save as .docx. The page styles above have gone

DOCX doesn't support the page styles if I remember right.

Just for double checking, Mike?

Yes, you are absolutely correct. Both Word and Excel only can configure page settings ad hoc, and have no concept of page styles (a named set of settings, that one can prepare, and then apply at will).

NOTABUG.

Why is it impossible for your users to leave styles in docx so that you can customize your documents more than once and immediately, and then.
And then it turns out that I have prepared cool accurate templates that have certain attachments, and they stupidly cannot be controlled.
And to explain to each person just working with the style of the page is stupidity, to say here is a template use it - THAT's IT.

Because we are not talking about "our" users, but about a specific file format. DOCX and XLSX are file formats created by Microsoft, and designed to hold everything that Word / Excel can create. There is *nothing* in the file format for features like page styles. Neither for the styles themselves, nor to tell which style is applied to which page.

Your question is like requirement to save page styles in TXT files. File format defines what data can be stored in it.
