Way to reproduce: Create a new empty calc document. Choose forms design mode, create checkboxes, modify them to have no label, exit forms design mode, check some of them. Export to pdf or print using windows' pdf printer. (formfields-test1.*) Result: only one checkbox is in the pdf, and shifted from its correct position. Modify the spreadsheet (formfields-test2.*) Export to pdf or print using windows' pdf printer. Result: no checkbox is in the pdf.

This was a very helpful comment. We have two PCs, each one running the same version of LibreOffice (24.2.1.2). The one showing the problem is a new one running Windows 11. On the other one running Windows 10, the exported pdfs are just fine!
I made some more tests. By exporting the spreadsheet to pdf on Win10, the resulting pdf is OK. By exporting the same spreadsheet to pdf on Win11, the resulting pdf is wrong. On both systems, the same version of LibreOffice is running, and the same version of the pdf viewer. The pdf exported on Win11 displays wrong when viewed on both systems, and the pdf exported on Win10 displays correct when viewed on both systems. Hence it is not a matter of the pdf viewer.
Please verify if there are the same options for pdf export on both systems.
- yes, same options for pdf export on both systems. - in the pdf created by direct export from LibreOffice, there is only one checkbox, shifted up. - printout from LibreOffice to a physical printer (HP Officejet Pro 8100) looks identical to the printout of Windows Print to PDF and Windows print to OneNote (only one checkbox, but shifted down a little bit). - in a print to pdf via PDFXChange Lite there is no checkbox at all.
To make sure that the problem is only in LibreOffice, I saved the document as xlsx and opened it in Excel (2021) and also in Planmaker (2024) on the same PC. I first had to set the background of the checkboxes to no color. Printing then is correct from Excel as well as from Planmaker.

I searched for "formfield" and "checkbox", not for "form objects" "Please try with LO 24.2.2.2" In 24.2.2.2 the problem is gone, thank you very much!
