1. Insert - Field - More Fields (Ctrl+F2)
2. Variables - User Field
3. Format - Additional formats - Date - OK

It seems the listbox of date formats pushes the Name and Value fields out of view.

Found the relevant report via Bugzilla search and confirmed with linux-64-24.8 to be 8e86df886f84fe69f13cfc367a5dd843e6ea917c
