Confirmed on a current build of 4.4.0.0.alpha0+ and Version: 4.1.3.2. It's an issue with both the right-click context menus and the toolbar in 4.4. Interestingly, if you accept or reject, the change list is properly traversed.
Confirmed on 4.4.1.2: when one change is next to another (ie., no unmodified text between them), as in the attached file, the next change is skipped. This bug also affects the new behavior of "accept change" and "reject change", which jumps automatically to the next change -- sometimes skipping changes that can remain unnoticed by the reviewer.
