3) Click 'Next Change' 3 times and you will see that it skipped the 3rd change and jumps to the 4th
Tested on 4.0.6 and 4.2.6 and master. The uno function was introduced in 4.0.

Confirmed on a current build of 188.8.131.52.alpha0+ and Version: 184.108.40.206. It's an issue with both the right-click context menus and the toolbar in 4.4. Interestingly, if you accept or reject, the change list is properly traversed.
Confirmed on 220.127.116.11: when one change is next to another (ie., no unmodified text between them), as in the attached file, the next change is skipped.
This bug also affects the new behavior of "accept change" and "reject change", which jumps automatically to the next change -- sometimes skipping changes that can remain unnoticed by the reviewer.

If you want to do more to help you can test to see if your issue is a REGRESSION. To do so:
1. Download and install oldest version of LibreOffice (usually 3.3 unless your bug pertains to a feature added after 3.3)
2. Test your bug
3. Leave a comment with your results.
4a. If the bug was present with 3.3 - set version to "inherited from OOo";
4b. If the bug was not present in 3.3 - add "regression" to keyword
