Could be related to 76257 When using a Calc sheet as a database Writer ignores the second line when printing ODTs in single-file-mode. Furthermore, one has to specifically tell Writer which entries to print if you choose single-file-mode and PDF as output file type. If one chooses "all", Writer only prints the first DB-entry - but for every line in the DB. Tested on Ubuntu 14.04.3 64bit
Please attach all example files so we can test. Set to NEEDINFO. Change back to UNCONFIRMED after you have provided the files.
The DB contains personal data - I cannot post it here in public. I would send them to you directly PGP encrypted but wasn't able to find your Key / any key for "Beluga"s E-Mail address (checked on https://pgp.mit.edu/). Could you please let me know how and where to obtain your key?
You could anonymize the data and provide such a version: https://wiki.documentfoundation.org/QA/Bugzilla/Sanitizing_Files_Before_Submission#Sanitize_file_text

Nothing is ignored in my single file ODT and PDF tests. All the 5 entries from the ODS get included. In the PDF the first entry is not repeated. Everything is like in the ODT. Perhaps I need to test this on Linux as well. For some reason you have included multi-file outputs in folders 1run_as-pdf_BUG and 2run_as-odt_WORKS. What are they supposed to demonstrate? Win 7 Pro 64-bit, Version: 5.0.1.2 (32-bit) Build ID: 81898c9f5c0d43f3473ba111d7b351050be20261 Locale: fi-FI (fi_FI)
Everything ok in Linux as well. Kubuntu 15.04 64-bit KDE Plasma 5.4 Version: 5.0.1.2 Build ID: 00m0(Build:2) Locale: en-US (en_US.UTF-8)
