LibrieOffice Base 4.4.2.2 crashes when opening form in Ubuntu 15.04

When using LO base 4.4.2.2 with Ubuntu (64 bit) 15.04 the data base opens fine, but when a form is opened LO Base crashes completely with the message ( Sorry Ubuntu 15.04 has experienced an internal error. If you notice further problems, try restarting the computer. )

Tested with several machines and same database.  It contains contact information, that's about it, so it's not a big form to open.

The same database was opened with LO Base 4.0.2.2/Ubuntu 14.04 (32 bit) and did not crash, we were able to open the form and add information.

@Rach : please indicate whether this is TDF LibreOffice or a version of LibreOffice provided by Ubuntu or a PPA.

If you have a PPA or Ubuntu supplied version, please retest with a TDF version of LibreOffice, and report back your findings here.

If the problem is clearly identifiable against a PPA/Ubuntu supplied version, please file a bug report in the Ubuntu Launchpad bugtracker.

1) Do you reproduce this on last stable LO version 5.0.3? (see https://launchpad.net/~libreoffice/+archive/ubuntu/ppa)

Crashes no longer occur with the 5.0.3 update
