In form design in Base, trying to add a form control, there is no icon for PUSHBUTTON

In my iMac install, the icon is there for forms in Writer, Calc, Impress and Draw. I have also installed LibreOffice on a Windows 7 laptop and the pushbutton icon is available as a form control in Base.

Please show the following as screenshot:
Mouse on top of the toolbar > visible buttons
Your screenshot shows 13 buttons - must be 15!

Icon for push-button hasn't been gone here with OpenSUSE 42.1 64bit rpm Linux, LO 5.1.2.2

This is a full screen shot with toolbars Form Controls and Form Design selected. If there are 2 buttons missing, I have just noticed that the Check box button is missing, as well as the Pushbutton button. 
All buttons are available when compared with Version 5.1.1.3 running on a Windows 7 laptop.

Sorry, my apologies. I have just done a side-by-side comparison of form editing on a Windows 7 laptop and an iMac (OS El Capitan 10.11.4). Down the left hand side list of buttons the iMac has two buttons missing compared to the laptop - PUSHBUTTON and LABEL FIELD (not the Check box button stated in my previous comment)

Can you attach the file you used for download to replicate?

Finally solved the problem!
In Form Design mode, if you right click in the list of buttons on the left, there is a menu item "Visible Buttons". Select that, and lo and behold - the push button and label field buttons have been de-selected! Who knew that was possible?
Select them both, restart the form design, and there they are.
Problem solved.
