There is an error with the preview of "Insert Index or Table of contents" dialog; it is not always correct and, more importantly, doesn't display all levels in the index the user selected.

Now I have to fiddle A LOT clicking 'OK' before I can see the actual effect of the new TOC settings. To make the process a lot easier, I would introduce an 'apply' button.

This also is consistent with all other styles & formatting dialogs.

Please give more details and ideally attach an example. "There is an error..." is not sufficient.
