Document that crashes LO 4.1.0.4 when inserting an index.

Problem description:

I have a document with an index, and with Libre Office 4.1.0.4 I can no longer update the index (Tools > Update > Update All) because it immediately crashes Libre Office. It seems to be the one document where I have this problem.

I have deleted almost everything from the document, including the index itself. Now it contains just one comment. Trying to insert a new index causes Libre Office to crash.

Steps to reproduce:
1. Load the attached Libre Office document (IndexCrash.odt).
2. Insert > Indexes and Tables > Indexes and Tables…
3. Press OK.

Current behavior:

1. Libre Office crashes. Running from the Terminal shows that the exit code is 139, but displays no error message.

Expected behaviour:

1. The index is inserted into the document.

Extra information:

If I delete the comment or change the format of the first line, then I can continue. Unfortunately, this does not help me in my original document where this problem originated.

Operating System: Linux Ubuntu 12.04 64-bit fully updated.

Libre Office version 4.1.0.4 downloaded and installed directly from the official LO website.

I can reproduce on 4.1.0 but it has since been fixed ( 4.2 master is fine) because of this I am marking as WFM

I recommend trying 4.1.1 as soon as it is released - if you get the crash there please mark this bug as NEW and we'll try to figure out what patch in 4.2 fixed the problem so we can backport it. Thanks!

@JoelMadero — I have tested this on LO version 4.1.1.2 (released today), and I am happy to report that it works correctly.
