When you rename a theme or an item in a theme, the old name is retained. Closing and opening the document again does not update the name of the item in the specified topic. In this case, the new name of the element (when you try to rename it again) is already established. But the old name is all the same.

The problem was caused by the fact that frozen (and not displayed) program instances remained in the computer memory. Each process takes about 50-55 MB. The last time I saw such symptoms in version 3.2. In the latest versions of this disease was not.
