The problematic document
This DOCX file was created in Microsoft Word. The document has an image and one line of text in the header area. In Microsoft Word 2013 the text appears below the image, aligned to the center of the page. In LibreOffice the text is in the same line as the image, thus the text is pushed to the right side of the page by the image.
Steps to reproduce:
1. Open the attached DOCX file both in LibreOffice Writer and Microsoft Word.
2. See the result.
The text is pushed to the left side of the page by the image in LibreOffice.
The text should appear below the image like in Microsoft Word.

If you want to do more to help you can test to see if your issue is a REGRESSION. To do so:
1. Download and install oldest version of LibreOffice (usually 3.3 unless your bug pertains to a feature added after 3.3) from http://downloadarchive.documentfoundation.org/libreoffice/old/
2. Test your bug
3. Leave a comment with your results.
4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
4b. If the bug was not present in 3.3 - add 'regression' to keyword

Modified document shows that the problem is not specific to the header as it happens in the document body if copied over.
