What I did:
I partially filled in a line on a form table.  There were some required elements that I hadn't filled in, and then I clicked out of the table, and the program crashed.

What I expected:
Mention that I failed to input all required information into the table on the form, then let me either leave or remind me to finish my entry work.

To help to reproduce this easily and quickly, could you attach an example odb file?
(do no forget to remove any confidential/private information from it).

I tried several times to reproduce it, and haven't had it re-occur with the same document.
I was unable to re-open the document, but after I closed Libreoffice and opened up the task manager... I found multiple copies of Libreoffice running in the background (several 100,000K + sized processes), I killed them and haven't experienced this particular crash again.

I've gone through and put gibberish into the table values.

When I unzip the odb file, I notice in the database folder there is a "backup".  Is this a concern, and is there any more steps I should take to sanitize the document?

To be honest I don't know.
Perhaps, you can remove the backup part of the file.
Of course, make a backup of the odb file this time before this action.

You don't need "backup". "backup" should contain the last data from opening the database last time. Might be it will have the same size when you close the database in LO and open it again, because there runs a script to minimize the content.

You could also delete "backup" for a file you will upload here.

Could you add please some default-data? The form is complete empty and without any data in the form nobody could test the behavior. The form with the error is a sub-sub-sub-form. So there must be data in the other forms to input data in sub-sub-sub-form.

Found out if you delete the "backup" in the db folder, that gets rid of the appearance of all the data in the dbase.  Is libreoffice recovering from the backup each time?  Oh well, here's a file with data in it.

CPU threads: 4; OS: Mac OS X 10.13.3; UI render: default; 
Locale: fr-FR (fr_FR.UTF-8); Calc: group

I tried adding data to each of the forms, either completely or deliberately leaving things blank. Each time where a required data point was required that I hadn't provided, I got an error message when attempting to move to the next record. The error message indicated the field of the missing data point to be provided.

Could you please try to reproduce it with the latest version of LibreOffice
from https://www.libreoffice.org/download/libreoffice-fresh/ ?
I have set the bug's status to 'NEEDINFO'. Please change it back to
