Please:
1.attach sample ODT which is per this report wrongly saved to DOCX
2.if you can ,add comparison screenshot of ODT in LO, DOCX in LO and DOCX in MSO
3.just to have complete info , try also with saving to DOC and reopening in LO and MSO
4.see via search of already reported, if this is a delicate of older bug
5. to confirm the bug, it needs to be tested with daily master from https://dev-builds.libreoffice.org/daily/master/current.html

When you attach ODT set Unconfirmed. Other steps are optional but useful.

I have not been able to confirm with bibisect-linux-64-7.0 or 7.1. Everything looks identical when I round-trip Brosjyre - Cold War.odt as DOCX.

I can confirm that when opening comment 3's document, the second picture is misplaced compared to the ODT - even in LO 6.4. So this sounds like an export bug - if it can be reproduced.

Repro 7.4+ both for DOCX and DOC.
Seems like export in Linux is also wrong but different. There was a change in 43all.

So, culprit is the anchor. "To page" cannot be saved in Word so it changes to "to character". And that would be "Wont Fix". Problem is that image changes position.
I mark "Minor" becase image can be anchored to paragraph or else in ODT and than positioned and saved.
So let's consider a duplicate of bug 135581.
