Forms in Base currently display the Writer icon in the task bar (tested on Windows 10, probably affects other platforms in a similar manner). This can get confusing when you have a database with Forms open, as well as a Writer document and some other applications, and need to constantly switch between them. I haven’t tried Forms in a while and don’t remember what task bar icon they use, but they might be affected as well. (Presumably due to Forms, and possibly also Reports, relying on Writer code.) Icons may be a global design consideration, hence forms in Base are not the only use case. However, Base seems to be the only application which opens multiple windows. One option would be to use the standard Base icon. Another would be to use the gray generic LibreOffice icon, as other windows in Base (tables, queries) do. Another option would be to design a generic collection of icons for Base windows. Then my suggestion would be to stick with the purple base color and just vary the image inside the page icon: e.g. cylinder (“storage”) for the main window, and for other windows take inspiration from the icons displayed in the main app window. However, the most pressing UX issue is the Writer icon being used on Base windows.
