Description:
Importing the attached file to Libreoffice 7.3.0.3 (x64, on Windows 10), the text fields work, but the check-boxes don't. In order to obtain the desired effect, the user has to know the appropriate special character (Ballot Box with X) to emulate toggling the box.

Steps to Reproduce:
1. Open attachment with Libre Office Writer.
2. Try to toggle one of the checkboxes.

Actual Results:
The checkbox behaves like an ordinary character, with no interactive features.

Expected Results:
Clicking the checkbox should toggle its state.

It is not clear, if it is always ☐ and ☒ as in this document (when opened with Office 365), or if the creator of the form could have chosen to make it toggle to ☑.
