Did it happen after new installation? 
Wht if you remove that icon and pin again?

Yes after install. 
I have unpinned / pinned app same result. Icon is same even if running and not pinned.

Issue is gone after several restarts. I have no idea what caused icon to return. I will try to reinstall and report again.

This can be a specific new problem, which would still need to be reprodiced and confirmed, but I'd rather mark as a duplicate of annoying bug 76131.
