Attached an enlarge screen shots of the {Don't save, cancel, save} Save document? windows. In my opinion, the different look of the 3 buttons {Don't save, cancel, save}, when one of them is selected, compared to the 2 other not selected buttons, should be much more visually obvious. There should be greater visual impact for the selected button, compared to the current thin extra rectangle. I find it problematic for regular, particularly new, users. As well as an accessibility issue.

At first glance, the screenshot looks like a screenshot on Linux with the gtk3 VCL plugin, but can you you please provide the exact version information from "Help" -> "About LibreOffice"?

If this is actually gtk3, I'd expect that buttons adhere to the Gtk style/theme selected in the settings.

What do buttons in other Gtk applications, e.g. gedit look like?

It could be I have upgraded LibreOffice, and some of its dependencies, since the bug was filed. But the issue is still here.

Here, gedit has the same issue. Other than the exact labels, gedit {Close without Saving, Cancel, Save As...} pop up window, in the case of a file created from scratch, looks exactly as LibreOffice {Don't save, cancel, save} pop up window.

Thanks for testing and providing more details.

In this case, this is not a LibreOffice problem. LibreOffice adheres to what the Gtk library and the configured theme define, which is why you can also see the same behavior in gedit.

You can try different Gtk themes to see whether there's another one that better fits your needs. This should be configurable in the settings for your desktop environment, e.g. GNOME or KDE Plasma. (Or you can manually set the environment variable GTK_THEME=<themename> before starting LibreOffice, e.g. GTK_THEME=Breeze.)

For example, I'm attaching 2 screenshots with different Gtk themes selected, but the same LibreOffice version:
