The point is that the new menu is shown for most docs but the attached one.
Do you have an example Bugzilla attachment with which it works? I tried with a random one and it didn't show up.

So I'm asking for a file that works for you.

Any new document will be accompanied with 'Macro Manager' pulldown proposal. Unless one changes the User interface to NotebookBar variations

I can confirm that specifically with this file, the Macro Manager command is not available in the menu. However, note that if you unzip the file, it contains its own customized "menubar.xml", which explains why the command is not available.
Before seeing Rafael's comment I tried to figure out why this was happening and came up with: "Possibly the Macro Manager... command and the separator were manually removed from the Tools|Macros menu using the Customize dialog? For me, adding both of these and then saving the document results in them appearing the next time the document is opened."
