Create a Table of Contents. Insert→Indexes & tables. 3. Note that the "del squared phi" now reads "del two phi". Example file attached. This has already been reported a couple of years ago. I will submit a patch for this in about a month. I am currently retaining sub- and superscript as well as italics.
