The export was done with LibreOffice on Debian Unstable, so libreoffice-writer 1:3.5.4+dfsg-3 The original document was available here: https://joinup.ec.europa.eu/community/facilitators_community/document/tips-online-community-engagement Note: on the site the document is now replaced by a PDF version. But that PDF is not created by LibreOffice.
I did confirm this using LibreOffice 4.0.0.3 and LibreOffice 4.0.1.0+ on Ubuntu 12.04. In the first case, the Table of contents showed an empty 2 row table. In testing with the next version of LibreOffice, nothing showed up under the Contents. The pdf from the website shows the correct result.
I see no content at all in the TOC in LibreOffice 402rc1, 401, 365, 357 ...
