I found this during tests for "Bug 61685 - Error message instead of all EXTENSIONs updates list": 

Proceeding as per <https://bugs.freedesktop.org/show_bug.cgi?id=61685#c1> shows something looking like a checkbox in the updates list pane, but there should be an alert icon? At least I see one in Server Installation of "LibreOffice 3.3.3  English UI/ German Locale [OOO330m19 (Build:301) tag libreoffice-3.3.3.1] on German WIN7 Home Premium (64bit).

See missing Icon with Tango, Galaxy, Crystal theme, others not tested.

confirmed on Win7 64bit using LibO 4.0.4
that icon is missing in Oxygen and Hi-Contrast themes as well
I suspect it was accidentally removed during some of the icons clean-up that has been done during LibO development

If you want to do more to help you can test to see if your issue is a REGRESSION. To do so:
1. Download and install oldest version of LibreOffice (usually 3.3 unless your bug pertains to a feature added after 3.3) from https://downloadarchive.documentfoundation.org/libreoffice/old/

2. Test your bug
3. Leave a comment with your results.
4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
4b. If the bug was not present in 3.3 - add 'regression' to keyword
