Index design window, with dead buttons and empty field grid.

Problem description: 

Steps to reproduce:
1. Create a new HSQLDB database document, create a new table by manual or by fairy.

2. Create an index, by clicking the Index creator button on toolbar, click "New index" button, adding some fields, click 
"Close", and "Save", everything is work and OK.

3. Click the Index creator button again! Only the New index button enabled, the others (rename, edit, delete) are disabled and the fields grid is empty.

Current behavior:
Only the New index button enabled, the others (rename, edit, delete) are disabled and the fields grid is empty.

Expected behavior:
All buttons are enabled and the field grid shows the fields of highlighted index.

Latest known-working version:
3.6.6

Version the bug appeared:
4.0.4_RC1 and 4.1.0_beta2 have the same behaviour.

Seems to be a duplicate of https://bugs.freedesktop.org/show_bug.cgi?id=63539. Could you please have a look at the description of this bug?

It should be marked as a duplicate (see "Status"). I have done it.
