Reproducible with LO 4.0.3.0 - 4.1.0.4 under Win7x64. Miklos, could you take a look at it?
As of 4.2.0.4, it looks a bit better. Page 8 looks OK. Still, page 7 has the ark mislaced, and page 9 is empty.
